You must provide all requested information in the application. An incomplete application will be returned to you. Once a complete application has been reviewed, you will be notified about whether you can proceed to the next stage of the application process.

MobilityPLUS application

For residents of Kitchener, Waterloo and Cambridge.

The application contains two parts. You must provide all requested information in the MobilityPLUS application and have a health/disability professional complete an additional form. An incomplete application will be returned to you. Once a complete application has been reviewed, you will be notified about whether you can proceed to the next stage of the application process.

  1. MobilityPLUS application, completed by the applicant (the person who will be using MobilityPLUS) or their delegate
  2. Health/Disability Professional form, completed by a health/ disability professional who is competent to speak to the applicant’s disability(s) and their ability to take conventional transit. Appropriate professionals could include (but are not limited to):
    • Physician
    • Nurse practitioner
    • Psychiatrist
    • Psychologist
    • Optometrist
    • Physiotherapist
    • Occupational Therapist
    • Social Worker
    • Orientation and Mobility Specialist
    • Managers from pre-approved Adult Day Programs or Long-Term Care centres
    • Other professionals approved by the Region

To complete this form, you will need:
  • Access to a printer to print the completed application.
  • A copy of your birth certificate or senior citizen's card if you are applying because you are 65 or older.
  • A photocopy of your CNIB card, if you are registered with CNIB.
  • A completed Part B: Mobility Assessment Form if you are under 65 years of age and not registered with CNIB.
  • A copy of your certification if you have a service animal.

Visitors to the City of Cambridge, Kitchener, Waterloo or the Township of North Dumfries may register with GRT MobilityPLUS for a maximum of six months.

To complete this form, you will need:

  • Your registration number for specialized transit services in your home city/region, if applicable
  • A photocopy of your CNIB card, if you are registered with CNIB
  • A copy of your certification if you have a service animal

Transit Support Person application
This application must be completed by a health care professional with one of the following designations:
  • Licensed physician
  • Registered nurse practitioner
  • Certified psychologist/psychiatrist
  • Licensed physiotherapist
  • Registered occupational therapist
  • Licensed optometrist/ophthalmologist
  • Registered Social Worker with the Ontario College of Social Workers and Social Service Workers
  • Developmental Services Resource Centre Service Coordinator
  • LHIN Care Coordinator
  • Certified Orientation and Mobility Specialist

Residents of Wellesley, Wilmot and Woolwich Townships can register with Kiwanis Transit. Complete the application, print and sign it, and fax to 519-669-3124.

Assessment

As part of the application process, GRT may request that you come in for an assessment.  Assessments are conducted at:

  • GRT Transit Operations Centre, 250 Strasburg Road, Kitchener; or
  • Hespeler Community Physiotherapy & Rehab at 38 Queen Street West, Cambridge.

You will meet with a Regulated Health Professional who will conduct industry standard assessments based on the barriers you identified on your application. The assessment will take between 20 and 60 minutes. The information from both the assessment and your application will be reviewed to determine your eligibility. You will be notified of the decision by mail.

Expiry and Renewal

Your MobilityPLUS ID card has an expiry date printed above your photo. If you would like to extend your registration and continue using MobilityPLUS, please call 519-585-7597 ext. 7345 at least two months prior to the expiry date. You will be asked to update your information and may be asked to come in for an assessment.
