You must provide all requested information in the application. An incomplete application will be returned to you. Once a complete application has been reviewed, you will be notified about whether you can proceed to the next stage of the application process.

Complete applications will be reviewed within 14 calendar days from the day they are received. Please contact us at 519-585-7555 if you have not been contacted by GRT by the end of the 14-day period.

MobilityPLUS North Dumfries application
To complete this form, you will need:

This application must be completed by a health care professional with one of the following designations:
  • Licensed physician
  • Registered nurse practitioner
  • Registered nurse
  • Certified psychologist/psychiatrist

  • Registered Social Worker with the Ontario College of Social Workers and Social Service Workers

Assessment

As part of the application process, GRT may request that you come in for an assessment.  Assessments are conducted at either 250 Strasburg Road in Kitchener or 150 Main Street in Cambridge. You will meet with an Occupational Therapist who will conduct industry standard assessments based on the barriers you identified on your application. The assessment will take between 20 and 40 minutes. The information from both the assessment and your application will be reviewed to determine your eligibility. You will be notified of the decision by mail.

Expiry and Renewal

Your MobilityPLUS ID card has an expiry date printed above your photo. If you would like to extend your registration and continue using MobilityPLUS, please contact GRT Customer service 519-585-7555 at least two months prior to the expiry date. You will be asked to update your information and may be asked to come in for an assessment.
